Bluestem Village – a newly built state-of-the-art skilled nursing facility is designed with the Resident (villager) in mind that focuses on the needs of individuals in a home-like environment. We strive to create a nurturing environment for the elderly and provide care and love to a growing community.
We will have Four independent non-traditional homes accommodating 14 villagers per house for a total licensed capacity of 56 beds. We will implement a new approach to long-term care that allows the villagers to thrive in their home with private rooms, baths and provided skilled nursing services in a person-centered environment. Outdoor amenities include a secure exterior courtyard with covered patios and walkways for villagers and a beautiful view of the valley.

OVERVIEW OF THE ROLE:
The primary responsibility of your job position is to plan, organize, develop, and direct the overall operation of the Social Services Department following the current federal, state and local standards, guidelines and regulations that govern long term care. You are entrusted to ensure that the medically related and emotional social needs of the villager are met and maintained on an individual basis.

SKILLS AND KNOWLEDGE:
Develop, maintain, and evaluate, direct the social service programs.
Provide guidance to household staff on social service issues.
Ensure house personnel have everything they need to do their job.
Work closely with Administrator/Director of Nursing to ensure household needs are met.
Develop and implement policies and procedures for the identification of the medically related and emotional needs of the villager.
Interpret the social service policies and procedures to employees, villagers, visitors, government agencies, etc.
Assist in the scheduling of social service care plans and assessments.
Involve the villager/family in planning social service programs when necessary.
Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and challenges of the villager.
Participate in the admission, discharge, MDS coordination, etc for social services.
Maintain a quality working relationship with the medical profession and other health related facilities.
Coordinate social service activities with other departments.
Assist in arranging transportation when necessary.
Interview villagers, or family members, as necessary to obtain social history.
Develop and maintain a community and social services file of agencies and organizations and organizations that provide assistance to villagers.
Review and develop a plan of correction for social services deficiencies noted during survey inspections and provide a written copy of such a report to the NHA.
Train/mentor staff as necessary.
Maintain current written records of social service expenditures.
Demonstrate a passion for caring as evidenced by interaction with co-workers, villagers, families, and visitors.
Maintain and support a culture of compliance, ethics, integrity and a person-centered environment in the Bluestem model.
Maintain confidentiality and rights of villager, family, employee, and community information in compliance with PHI HIPAA policies and applicable regulations.
Maintain safe work environment and exhibits safe work practices.
Follow agency procedures when purchasing items on behalf of the agency.
Follow agency policies and procedures in carrying out duties.
Advise supervisor of concerns and problems that arise within the agency.
Safeguard all Bluestem Village funds and property, including those individuals in our care.
Performs other duties as assigned by the Supervisor, Administrator, and/or designee.
Other reasonable duties as may be assigned.
THE MUST-HAVES:
Must possess, as a minimum, a Bachelor’s Degree from an approved school of Social Work or a bachelor’s degree in a human service field including but not limited to sociology, special education, rehabilitation counseling, and psychology. Master’s degree preferred.
Must have, as a minimum, two (2) years’ experience or related field in a supervisory capacity in a hospital, nursing care facility, or other related medical facility
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the villagers served
Must be knowledgeable of laws, regulations, and guidelines pertaining to hospital and nursing community administrative procedures
Must be knowledgeable of computer systems, system applications, and other office equipment
Ability to pass a criminal background check as well as Colorado CAPS background check.
